RULES

The following rules (“Rules”) together with Communicorp UK’s ("Communicorp UK’s ") general and competition terms and conditions (the “Terms and Conditions”) (which can be found at www.capitalfm.com apply to ‘Deep Sea World Nov 2015’ radio competition’ ("the Competition") which will run on 20/11/15 in the drive show (“the Duration”). The "Radio Station" is 105 -106 Capital FM.

1Anyone who enters the Competition (an “Entrant”) will be deemed to have understood these Rules along with the Terms and Conditions and will be bound by them. The Terms and Conditions are available on request from the Radio Station.

Details of the Competition:

To enter the Competition an Entrant must call the studio on 03452 777 106, during the drive show on Friday 20th Nov. The opening and closing times of competition will be given out on air.
2.1 A caller will be selected at random and brought to air to play ‘Dive Time’

18s & over only.

Prize:

A Shark Dive Voucher + a Go Prod Camera

• Once a voucher has been recieved, you must then book a dive slot. Please call the aquarium on 01383 411880 to book your dive. We recommend booking at least eight weeks in advance.

MEDICAL QUESTIONNAIRE

Whilst the dive experiences are open to anyone, we do recommend that before purchasing a voucher you read through the following Medical Questionnaire. All customers are required to complete a medical questionnaire in advance of their dive. If you answer YES to any of the questions below, or are unsure of your answer to any of these questions then you must seek authorisation from your Doctor.
